Welcome to Ames Spine & Sport! Whether you schedule your first visit in our office, over the phone, or on our website or the Vagaro app, there is some information we will need from you before your appointment.
We start with the obvious information – we need your name as well as a good phone number and email address to contact you. If you scheduled on our website and entered this information yourself, someone will call you for the rest of the information we need.
Next, we will ask you a few additional questions:
- What is your date of birth? We keep electronic health records and our system requires your birth date as part of the sign in process.
- What are your primary complaints? i.e. why are you coming in to see us? For example: headaches, back pain, knee pain, etc.
- Do you want to go through insurance? As of January 2026, we are in-network with Blue Cross & Blue Shield PPO and Federal Employee Program plans, as well as Medicare. We can provide a superbill and member submitted claim form if we are out of network with your insurance. If you don’t have chiropractic benefits or our providers are out of network with your plan, we also partner with a third-party program called ChiroHealthUSA (CHUSA) that allows us to legally discount chiropractic care and helps make visits more affordable.
After we get this information from you, you can expect a couple of things from us:
- An “Appointment Reminder” email from Vagaro (our scheduling system) with an appointment confirmation. If you scheduled yourself online, you should have already received this email.
- A “Patient self check-in” email from our electronic health records system to complete your new patient paperwork. The log in information will be information we got from you when scheduling. The new patient paperwork is THOROUGH…bear with it. The more complete the information you enter, the better our providers will be able to help you. It takes most people 15-20 minutes to fill out. If you would rather fill out your new patient paperwork in our office, we have iPads we use for patient check in and you are more than welcome to use one of those. We just ask that you arrive at least 20 minutes prior to your appointment to allow you time to fill everything out and still have your appointment start on time.
